Ralph R. Velazquez, Jr., M.D., Chief Medical Officer, OSF HealthCare, has been employed by OSF since 1994. Dr. Velazquez’s responsibilities include building teams of healthcare professionals across the OSF care continuum. He is the Chief Medical Officer for the Pioneer ACO, responsible for Care Management across the ministry, oversees credentialing, is the chairman of the System Pharmacy and Therapeutics committee, supports performance-based contracting negotiations and supports health benefits design and operations.

A graduate of Monmouth College, Monmouth, Illinois, and the University Of Illinois College Of Medicine at Peoria, Dr. Velazquez completed his Family Medicine Residency at Methodist Medical Center in Peoria. As a member of OSF Medical Group, Dr. Velazquez was in practice at Rockford Family Practice before being recruited to serve as the Chief Medical Officer for OSF HealthPlans, Inc. in 1997. In 1998 he achieved his Certified Managed Care Executive status through the AAHP Executive Leadership MD program.

Dr. Velazquez completed a DHHS Primary Care Policy Fellowship in 2003, and became a Fellow of the American Academy of Family Practice in 2004. During 2005-2006, he served as the Caterpillar-OSF Quality Quest Co-Director. He serves on the Humana Clinical Care Committee and is board certified in Family Medicine, a member of the American College of Physician Executives, and of the Monmouth College Board of Trustees.
